The General Conference of the United Methodist Church established a welfare benefit plan for Clergy effective January 1, 1982. Wespath (a general agency of the United Methodist Church) maintains the Comprehensive Protection Plan, also known as CPP, for the benefit of Clergy of the United Methodist Church. The Claims Administrator for the disability benefits under the Plan is Liberty Life Assurance Company of Boston. Wespath has delegated the administrative authority to review and recommend approval or denial of Claims for disability benefits to the Claims Administrator.

To provide Wespath with information relative to the participant's application for long-term disability benefits.
Form C
To provide Wespath specific information relative to the clergyperson's ability to perform his/her usual and customary duties.
